TRT Monitoring: The Essential Blood Tests

You have felt sharper for a few months now, your recovery runs better and your training is biting. Naturally you think: measure testosterone, number up, done. But following TRT well is not about that single number. It is about a handful of values that together show whether your body can handle the programme, and that your doctor uses to adjust course before anything drifts out of balance.

The honest stance up front: anyone who only checks total testosterone and ignores the rest is optimising blind. A haematocrit creeping up, or an oestradiol going off the rails, is invisible in the mirror. You see it in your data, and only if you collect the right data.

Why one testosterone value says too little

Testosterone does not act in isolation. Part of it circulates bound to SHBG, and only the free fraction is biologically active. That is why a doctor often reads total testosterone and free testosterone together. Two men with the same total value can have a completely different experience purely because of their SHBG.

The Endocrine Society stresses in its guideline on testosterone therapy that dosing targets a trough value in the mid-normal range, not the highest achievable number. More is emphatically not better here. The Dutch College of General Practitioners (NHG) further notes that diagnosis and management of male hormone deficiency always belong with a doctor, not with self-steering on loose results.

The monitoring panel: what, why and how often

The table below summarises which values are common in TRT follow-up, why they matter and how often they typically recur. Treat it as a checklist you fill in with your doctor, not as dosing advice.

The frequency figures are guidelines, not law. Your doctor adapts the schedule to your baseline values, your age and how your body responds.

Haematocrit: the value you must not skip

If there is one marker TRT users structurally underestimate, it is haematocrit. Let it rise too far and your blood literally gets thicker. The Endocrine Society therefore advises a threshold above which treatment is reconsidered, often around a haematocrit of 0.54. If you hit that, your doctor looks at dose, delivery form or a blood donation as an option. This is not a value to interpret yourself with a table from the internet: it is exactly the moment a doctor should look along with you.

Timing makes your data usable or worthless

A common mistake is to draw blood at random moments and still compare results with each other. With injectable TRT your testosterone level swings over the days after a shot: a peak just after, a trough just before. That is why a doctor steers on the trough value, because it is the most stable and the most comparable between measurements. Draw blood two days after your injection one time and just before the next another time, and you are comparing apples with pears.

A practical line many doctors hold: with injectable forms, measure just before your next dose, and keep that moment identical at every check. If you use a gel or another daily form, a fixed time in the morning makes more sense. Note which form and which timing you use, so your doctor places your result in the right context. Without that context, even a perfect number is hard to read.

What a complete TRT check looks at besides hormones

Hormones are the core, but a good TRT check looks wider. Your liver and kidney function show whether your body processes the treatment well, and your lipids show whether your cardiovascular profile is moving along. Especially if you train hard alongside TRT and adjust your nutrition, those values can shift. A check that measures only testosterone misses exactly the signals you would have wanted to see in time.

What you do with this

Plan your measurements around a fixed point in your dosing cycle so your trough values stay comparable. Bring your previous results, because in TRT the trend tells you more than a snapshot. And discuss any pattern, a rising haematocrit, a drifting oestradiol, a PSA change, with your doctor before you touch your dose.
