Transferrin saturation: the iron value that counts when ferritin misleads

Transferrin saturation is the percentage of your transport protein that is actually carrying iron, usually between 20 and 45 percent. It tells you something ferritin does not reliably tell you in a hard-training body: how much iron is available right now.

That difference is not a detail. It is the most common way iron deficiency gets missed in athletes.

Ferritin is also an inflammation protein. During a heavy training block, hepcidin and ferritin rise as part of an acute phase response (PMID 32038278). Your stores can be draining while the number meant to measure them goes up.

What exactly is transferrin saturation?

Iron does not travel loose through your blood, it rides bound to a transport protein called transferrin. Transferrin saturation, also written TSAT, is simply what percentage of those transport seats is occupied. The lab calculates it by dividing your serum iron by your total iron binding capacity.

Read it as the occupancy rate of your transport.

On your result it appears as transferrin saturation, sometimes only as a percentage beside your iron and your ferritin.

What is a normal transferrin saturation?

Most Dutch labs use roughly 20 to 45 percent for adults, with a slightly lower bottom limit in women. Below 20 percent points towards too little available iron, and below 16 percent makes that clearer. The exact range differs by lab.

Unlike ferritin this is a ratio, and ratios are less sensitive to inflammation.

Why does ferritin mislead in athletes?

Because ferritin measures two things at once: your iron stores and your inflammation level. During intensive training, hepcidin rises, the hormone that blocks iron absorption, and ferritin rises with it as an acute phase protein. Research in elite junior rowers saw hepcidin climb during a period of high training load (PMID 32038278), and work in basketball players showed comparable shifts after acute exertion (PMID 35428332).

The result is a number moving the wrong way exactly when you need it most.

Imagine two runners aged 28 who both have a ferritin of 42 µg/l. One is training easily and has a transferrin saturation of 31 percent, the other is deep in a heavy block and comes out at 12 percent. Same ferritin, completely different iron picture.

Iron balance in female athletes is a research field of its own for exactly this reason (PMID 29280410).

How do you combine ferritin and transferrin saturation?

By never reading them separately, and by putting an inflammation marker beside them. If your ferritin is normal but your saturation is low, look at your CRP. If that is raised, your ferritin is almost certainly inflated and the low saturation is the more honest number.

Those three together decide more than any single number does.

A fourth signal completes the picture: your RDW. It rises in early iron deficiency before your MCV falls, so a raised RDW beside a low saturation is a strong combination (PMID 23715364). For that, read RDW blood value.

When should you draw for an honest iron picture?

Not during or just after a heavy training block, and not during or just after an infection. Schedule your draw after at least 48 hours without hard training, fasted, in the morning, because your serum iron follows a clear daily rhythm and falls later in the day.

Draw on a random afternoon after a double session and you are mostly measuring your week.

What does a high transferrin saturation mean?

A saturation above roughly 45 percent, and certainly above 50, can point to too much iron rather than too little. That happens with excessive supplementation and with haemochromatosis, an inherited condition in which you absorb too much iron. It becomes visible earlier in men than in women.

This is one of the few blood values where the top end is more serious than the bottom end.

If you take iron on your own initiative and your saturation reads high, do not simply carry on supplementing, discuss the result with your GP.

When should you discuss an abnormal value with your GP?

With a saturation below 20 percent alongside fatigue, breathlessness during exertion or declining performance. Also with a saturation above 45 percent, certainly if iron overload runs in your family, and always before you start iron supplementation yourself.

Thuisarts describes an abnormal blood result as a reason to look further rather than a diagnosis, and the NHG anaemia guideline approaches iron deficiency through the pattern of several values together. RIVM points out that reference values differ per laboratory in blood testing, which matters for a calculated value like this because labs determine iron binding capacity differently.

Taking iron without a result is not a good idea. Too much is a problem of its own, not a harmless surplus.

Where do you start?

Ask for your transferrin saturation explicitly alongside your ferritin, because many standard panels only report the latter. Put a CRP beside it so you know whether your ferritin can be trusted. Draw after 48 calm hours, fasted and in the morning.
