You do everything right: you train hard, you eat well and you sleep enough. Yet your times slow and you feel flat. Before you train even harder, the question that matters: are you training too much or recovering too little? Your blood gives hints, but less firmly than many people think.
My take: there is no overtraining test. Anyone selling one blood value as proof of overtraining is selling false certainty. The pattern over time, together with your symptoms, is what counts.
Why one marker is not enough
Overtraining, often called overtraining syndrome in sports medicine, is a state of persistent underperformance despite rest. The international sports medicine bodies describe it as a combination of symptoms and markers, not as a cut-off. That means no single result makes the diagnosis, and a normal blood picture does not rule out a problem.
The value of blood testing here is mostly in ruling out other causes, such as iron deficiency or a thyroid issue, that look like overtraining but are not. In other words: blood rules out suspects, it rarely points to the culprit directly. That is not a shortcoming of the test, but the nature of overtraining: it is a diagnosis by exclusion, made over time.
The three markers and what they do and do not say

Cortisol fits hard training; a raised value is not an alarm on its own. CK always spikes after heavy or unusual load and mainly says your muscles are damaged, not that you are overtrained. CRP and hs-CRP point to inflammation, but a cold or an injury drives them up too.
The pattern that does matter
You recognise overtraining by the combination. A resting heart rate that stays raised for weeks, sleep that worsens, irritability, and performance that does not return despite rest. Add a persistent mild inflammation and a high cortisol and the picture strengthens. None of those signals stands alone.
The Voedingscentrum and the Gezondheidsraad stress, for sport, the importance of adequate energy intake and recovery; eating too little while training a lot causes similar symptoms. So first separate under-recovery from a true overtraining syndrome.
That distinction matters more than it seems. Under-recovery is functional: a few hard weeks followed by extra rest belong to a well-built season, and you bounce out of it stronger. A true overtraining syndrome is a different story; recovery then takes weeks to months and the solution is to scale back sharply, not to push through. Anyone who misses the difference often keeps training exactly when rest is needed.
How often you should measure CK and CRP
Single readings of cortisol, CK and CRP are hard to interpret, precisely because they swing strongly with your training. A repeat measurement at rest gives far more information. If you measure CK two days after a hard session and it is still markedly raised, you are recovering more slowly than expected. If your hs-CRP stays raised across several rest measurements, that is a signal to reconsider your load. A single spike right after effort says almost nothing.
What mimics overtraining
Before you think overtraining, rule out the ordinary causes. These three look strikingly similar:
- Iron deficiency: read iron deficiency in athletes.
- A thyroid issue: an off TSH or free T4 gives similar fatigue.
- Low vitamin B12 or D: read tired despite good training.
When to rest rather than test
If you are unsure between overtraining and under-recovery, a planned recovery week is often more informative than a blood test. If everything recovers with rest, it was probably under-recovery. If symptoms persist after a real recovery period, blood testing is useful, mainly to rule out other causes.
What you watch alongside blood costs nothing and often says more. Your morning resting heart rate is a sensitive gauge: if it stays raised for days, you are not recovering enough. Your sleep quality, your mood and your appetite for training are signals too, ones that tip earlier than any marker. Keep them in a simple log for a few weeks and you see a pattern emerge that a single blood test misses. Only when that log and your gut keep pointing to a problem does blood testing really add something.
