Gut flora and athletic performance: what a microbiome test does and does not say

Your gut flora is the collection of bacteria in your intestine, and in athletes it looks different on average than in non-athletes. That is real research, not marketing. Whether a microbiome test costing 150 to 250 euro lets you act on it is a very different question.

My position: the science is genuinely interesting, the consumer test lags a long way behind it.

What is your gut flora and what does it do?

Your gut flora, now usually called the microbiome, is the whole population of bacteria and other micro-organisms in your intestine. They help break down fibre, produce short-chain fatty acids such as butyrate, and take part in your immune defence. The composition varies widely between people and shifts with what you eat.

That person-to-person variation is immediately the awkward part. There is no single reference composition everyone should be aiming at.

Fibre is the main dial you can turn yourself. The Dutch Nutrition Centre, Voedingscentrum, works with a guideline of roughly 30 to 40 grams of fibre a day for adults, and most people in the Netherlands sit well below it.

Do athletes have different gut flora?

Yes, and the best known example comes from research on marathon runners. Researchers found more bacteria from the genus Veillonella in stool samples after the Boston Marathon, and that bacterium lives on lactate. In mice, giving Veillonella atypica led to longer run times (PMID 31235964).

It is a great story and it routinely gets oversold. An effect in mice is not a performance gain in you.

A review by sports nutrition researchers concludes that exercise, diet and environment together shape the composition, but that cause and effect are hard to separate (PMID 32398103). Athletes also eat differently, sleep differently and move more.

What probably counts most is the fibre intake that tends to come with an athletic diet, not the training itself.

What does a microbiome test actually say?

A consumer microbiome test uses DNA analysis of your stool to map which bacterial groups are present and in what proportion. That part is technically sound. Translating it into personal dietary advice is where the evidence thins out, because there is no agreed standard for a healthy microbiome.

Think of it as a photo of a busy square. You can see who is standing there, you do not know why they came or who will be there tomorrow.

Repeat the test two weeks later after a holiday or a course of antibiotics and the picture can have shifted considerably. That makes a single measurement a shaky basis for lasting choices.

Do probiotics help athletes?

The effect is sometimes measurable, but small and heavily strain-dependent. Research focuses mainly on fewer gastrointestinal symptoms during endurance exercise and on upper respiratory infections. What works for one bacterial strain does not automatically apply to another tub on the same shelf.

That makes most general claims on packaging hard to stand behind.

One thing that keeps returning in exercise research: during hard training less blood reaches your gut and permeability rises temporarily, which can explain cramping and urgency (PMID 28589631). That is a load problem, not a bacteria problem.

What can you actually influence?

Fibre, variety and recovery. More plant variety across a week is associated with a more diverse microbiome in observational research, and fibre is the fuel for the bacteria that make short-chain fatty acids. That costs no test, only a shopping list.

In practice: more different vegetables, pulses, nuts and wholegrain products spread across the week.

Build it up gently if you currently eat little fibre. Going from 15 to 35 grams in a single week produces exactly the symptoms you are trying to fix.

Which blood values are relevant here?

A microbiome test and a blood test measure very different things, and they do not replace one another. Blood work can show whether long-running gut symptoms or a narrow diet have affected your stores and your inflammation markers. That is information your GP can genuinely use.

Values often included here are ferritin, vitamin B12 and CRP. A broader panel such as 360 Health covers them in a single draw.

What I would do

I would not spend the 200 euro on a microbiome report, but on four weeks of deliberately more fibre variety and an honest food diary. If symptoms persist after that, discuss them with your GP before cutting food groups on your own.
