Understanding your testosterone levels

Your result is in and there is a number next to "testosterone". But is 16 nmol/l good? And why does the GP put a tick next to it while you feel terrible? A number without context is almost useless. Here you learn to read your own result.

My conviction: the laboratory measures, but you only interpret together with your doctor. A value "within range" is no guarantee that everything is fine.

What is a normal testosterone value?

Reference ranges differ per laboratory and per measurement method, so always read the range printed on your own result. Still, a rough framework helps. The table below gives commonly used orientation values for total testosterone in adult men. They are guide values, not hard limits.

Notice the table gives no exact figures? That is deliberate. The spread between labs is wide enough to be misleading if you memorise a fixed number.

Total, free or SHBG: what do you read first?

A result starts with your total testosterone, but that number does not tell you how much hormone your tissues can actually use. For that you look at your free testosterone and at SHBG, the protein that binds testosterone. A high SHBG can make a perfectly normal total work out low.

Which other values should your doctor read alongside it?

A testosterone result never stands alone. Anyone who sees a low number also wants to know where it comes from. That is why a few values belong with it that complete your picture. LH shows whether the signalling from your pituitary is right. Estradiol belongs because part of your testosterone is converted into it. And a raised prolactin can explain a low testosterone that you would otherwise miss.

That is exactly why a single testosterone number gives you little. Only in context does it tell you something usable. A good panel delivers these values in one draw, so your doctor has the whole picture instead of loose puzzle pieces.

Common reading mistakes

When interpreting a result, things often go wrong in the same way. The mistakes I encounter most:

  • Comparing a number with the wrong range: reference ranges differ per lab. Always compare with the range on your own result.
  • Taking an afternoon draw seriously: a low result in the afternoon can be purely down to the timing.
  • Looking only at the total: without SHBG and the free fraction you miss half the story.
  • Treating one measurement as truth: you repeat a deviating result before drawing conclusions.

My rule of thumb: a number is only a fact when it is repeatable, drawn at the right time and fits how you feel.

When does a number really mean something?

A value only becomes meaningful when you account for three things: the time of the draw, repeatability and your complaints. Testosterone peaks in the morning, so an afternoon measurement underestimates your value. The Dutch College of General Practitioners (NHG) therefore advises a morning draw and a repeat when a result deviates.

The Endocrine Society adds that an isolated low number without complaints is rarely a reason to intervene. The interplay counts, not the single figure.

What if your value is "within range" but you do not feel well?

This is perhaps the most frustrating situation: your number gets a tick, but you feel terrible. Important to know: a reference range is a statistical range, not a personal optimum. The bottom of the range can be fine for one person and just too low for another.

In that case it pays to look beyond your total testosterone alone. A normal total with a low free testosterone and a high SHBG can still explain your complaints. Other causes belong here too: a slow thyroid, an iron deficiency or poor sleep produce similar complaints. A result "within range" therefore does not rule out a problem, it only shifts the question to where you should look next. You have that conversation with your doctor.
