Quarter to six. The alarm went off, you have not eaten a thing, and you are on the mat ready for your cardio. No oatmeal, no banana, just black coffee and the idea that your body now has no choice but to tap into fat. It feels clean, almost heroic. But what actually happens in your blood at that moment, and does it deliver the fat burning and performance you are hoping for?
Fasted training simply means working out without recently eating carbohydrates, usually early in the morning or after an overnight fast. The mechanism behind it is logical: your insulin is low, and low insulin gives your body room to release and burn fatty acids. The only question is whether that translates into less fat on your body. That is where the nuance sits.
What happens in your blood during a fasted session
During fasted exercise your whole metabolic landscape shifts. Because no fresh glucose arrives from a meal, your body leans more on stored fuel. Below you can see, per marker and hormone, what typically changes and what it can mean for you.

Does fasted training really burn more fat?
Here is my honest stance: yes, you burn more fat as fuel during the session, but that does not automatically mean you lose more fat net. What counts over 24 hours is your total energy balance and your total protein intake, not which fuel you happened to tap at quarter to six. Your body compensates later in the day. Anyone training fasted purely because the fat-burning app shows a higher number is chasing the wrong number.
That does not make fasted training worthless. For easy low-pace endurance work it can train your fat metabolism, and for many people it is simply practical: you do not have to eat first. The gain is flexibility and adaptation, not a magic fat burner.
Effect on your insulin sensitivity
Moving regularly, fasted or not, can improve your insulin sensitivity: your cells respond more sharply to insulin and you keep your blood sugar more stable. The flip side: cortisol, recovery and muscle breakdown
Fasted training raises cortisol and catecholamines. That is functional: it delivers energy. But on long or very intense sessions, or if you structurally undereat, chronically high cortisol can undermine your recovery and encourage muscle breakdown. For high intensity such as intervals, strength training or long endurance blocks, the performance loss from empty glycogen stores outweighs the extra fat burning. There, training fed is almost always the smarter choice.
Who does it work for, and who not?
- Can fit well with: easy low-pace cardio, short sessions, people who find fasted training comfortable and practical.
- Better not fasted: heavy intervals, high-volume strength training, sessions longer than an hour at intensity, and anyone prone to dizziness or fading energy.
- Extra careful: women can respond more sensitively to the hormonal stress of prolonged fasted training; effects on cycle and hormone balance vary a lot per person, so listen to your body and adjust where needed.
In the Dutch physical activity guideline, the Gezondheidsraad (Health Council) mainly stresses that you move enough and regularly; the exact timing around meals is secondary to that. None of these bodies claims that fasted training is necessary for a healthy metabolism.
How to test with blood values whether it suits you
Feel is subjective; your blood is not. A few markers show how your body responds to fasted training. Your cortisol gives an impression of your stress load, your HbA1c shows your average blood sugar over weeks, and your IGF-1 tracks with your growth hormone and recovery axis. Combine that with fasting glucose and insulin and you get an honest picture instead of a gut feeling.
If you train hard or are tuning your hormones, it can be wise to look wider with general hormones or, if you use TRT, with TRT monitoring. See these values as a dashboard, not a verdict, and always discuss anything off in consultation with a doctor.
