Testosterone and muscle mass: what strength training does to your hormones

You have just wrapped a heavy squat session and feel like a titan. The idea that this training drove your testosterone sky-high feels logical. Is it true? Partly. But the brief spike after training and your actual daily testosterone level are two different things, and only the second builds muscle over the long run.

Here is the stance that pulls a lot of gym myths out from under their feet: you do not build a permanently higher testosterone with isolated heavy sessions. What really steers your baseline is lifestyle, sleep, body composition and recovery. The acute spike is fun, but not where the gain is.

What strength training does acutely

Heavy compound exercises with plenty of volume give a measurable rise in testosterone shortly after training. That spike is real, but temporary: within hours you are back at your baseline. The idea that you permanently lift your daily testosterone by training ever heavier is not supported by the data.

More important for muscle growth is that your training stimulus makes your muscles more sensitive to the hormones you already have. You build muscle mainly through consistent loading, enough protein and recovery, not by chasing a hormone spike.

What really sets your daily level

Your baseline testosterone, the number that matters, hangs on factors you largely control yourself. The table below sets out the biggest levers.

The message is freeing: you need no exotic tricks. The biggest gain comes from sleep, body composition and smart recovery. The Netherlands Nutrition Centre (Voedingscentrum) stresses that a balanced diet and a healthy weight form the basis for your hormone balance, and that base does more than any pre-workout.

An honest word about expectations

If you have a normal testosterone level, training will not raise it dramatically, and it does not need to in order to get stronger. If you have structurally low values with complaints, that is a medical question, not a training question. The Health Council of the Netherlands (Gezondheidsraad) and the Dutch College of General Practitioners (NHG) stress that persistent fatigue or low libido belong with a doctor, not with self-experimenting on agents that are supposed to boost testosterone. TRT is a medical treatment, not a training upgrade.

Why free testosterone tells the whole story

A common error in thinking is that your total testosterone says everything about your muscle potential. But a large part of that testosterone is bound to SHBG and therefore not active. It is your free testosterone that can actually get to work in your muscle cells. Two athletes with identical total testosterone can have a very different free level because of a difference in SHBG, and so a different experience of energy, recovery and strength. Anyone who looks only at the total number misses that nuance entirely.

That also explains why a low total number is not always a problem, and a tidy total value is not always reassuring. It is the free fraction and your SHBG together that give you the usable picture. Discuss that with a doctor and you can tell a genuinely low value apart from one that simply falls within your personal normal.

You build muscle with the hormones you have

The reassuring truth is that you do not need elevated testosterone for muscle growth, as long as your values fall within a healthy range. Progressive overload, enough protein and consistent recovery do most of the work. Testosterone is a contributing factor, not a magic button. Anyone who has their sleep, nutrition and training planning in order almost always gains more than someone fixated on a hormone number. That is not an anti-data message, it is reading the data honestly.

What you do with this

To know where you stand, measure your testosterone in the morning, when your values are naturally highest, and place it next to your SHBG and free testosterone for a complete picture. A broader hormone panel is at general hormones, and your heart and metabolic markers at lipids.
