Ashwagandha reviews: what users report versus what trials measured

Ashwagandha users most often report feeling calmer, falling asleep faster and worrying less. The trials show that in part, but not everywhere. In the one trial that measured how men actually felt, there was no significant difference against placebo on fatigue, vigour and wellbeing.

That is an awkward combination, and that is exactly what makes it interesting. Thousands of people report something, and the measurement does not find it.

What do users report most often?

Four things recur in Dutch user accounts: feeling calmer under stress, falling asleep more easily, fewer racing thoughts in bed, and in some people a flatter feeling during the day. That last one is rarely presented as a benefit.

People describe it as feeling less stress, but also less of everything else.

Those four points are not a research finding. They are what people write down, and that is a different thing from a measured outcome.

What are the benefits according to the trials?

The measured benefits sit mostly in stress questionnaires and in cortisol. In 64 adults with chronic stress, scores fell on every stress scale, and serum cortisol was lower than placebo at p=0.0006 (Chandrasekhar, 2012). A second study in healthy adults found comparable effects on stress and anxiety (Salve, 2019).

In lifters the gain sat somewhere else entirely: in strength, muscle size and testosterone (Wankhede, 2015).

Note who was measured. All three groups differed sharply, and that explains part of the conflicting stories.

Why do reviews and trials disagree?

The sharpest test is a crossover study in 57 men aged 40 to 70 with mild fatigue. They took a standardised extract for eight weeks. Salivary testosterone rose 14.7 percent and DHEA-S 18 percent, both significant (Lopresti, 2019).

But on fatigue, vigour, mood and sexual wellbeing there was no significant difference against placebo.

Read those two sentences side by side. The hormones moved, the feeling did not. That is the exact opposite of what most user accounts describe.

Take two men of 45 both taking the same extract for 8 weeks. One feels calmer, the other notices nothing, and their hormone values can move in exactly the same way.

The honest explanation is that expectation carries much of the effect. You buy a jar, you start paying attention to your sleep, and you sleep better. That is real, just not necessarily because of the herb.

The bottom row is the most honest line in the table. What nobody measures, nobody can confirm or deny.

What does ashwagandha do to your hormones?

What has been measured is mainly cortisol, testosterone, DHEA-S and the thyroid hormones. The direction differs per study and per group, which makes it hard to say what it does in you. In men aged 40 to 70, for instance, cortisol did not change significantly, while in stressed adults it fell.

The thyroid is the least discussed effect. In a trial in subclinical hypothyroidism, TSH, T3 and T4 all changed (Sharma, 2018).

Why not use ashwagandha?

On 5 March 2024, RIVM advised against supplements containing ashwagandha, and especially against use in pregnancy. The trigger was reports from doctors about poisonings, including harmful effects in the liver. That advice is separate from whether you notice anything.

Positive experiences say nothing about the rare serious cases.

What you can check yourself

My own line is simple. If you want to know whether it does anything in you, a feeling is a weak yardstick. That is not a criticism of you, it is what placebo research has shown for decades.

A cortisol test before you start at least gives you a reference point. If the difference you notice is mostly in training, total testosterone is the matching value.
