Urine Leukocytes

Leukocytes in urine are white blood cells detected in a urine sample. In a healthy adult the expected result is negative. A dipstick does not measure the cells directly but leukocyte esterase, an enzyme released by neutrophil granulocytes. That is why the pad can stay positive once the cells themselves have broken down. A positive result means immune cells are active somewhere in the urinary tract, but on its own it does not prove a urinary tract infection. Leukocytes in urine are also entirely separate from the leukocyte count in blood: a different test, a different sample, a different meaning.

Urine Leukocytes: co mierzy to badanie

The test shows whether white blood cells are present in urine. This is done in two ways, and they do not measure the same thing.

The dipstick carries a pad for leukocyte esterase, an enzyme from the granules of neutrophil granulocytes, the white cell most abundant in an acute inflammatory response. The pad changes colour once that enzyme is present. It does not see the cell; it sees what the cell leaves behind. That explains a result which often causes confusion: the strip can read positive while the laboratory finds few or no intact cells under the microscope. The cells have already broken down, for example in very dilute urine or in a sample left standing too long. This is not a measurement error. It is exactly what an enzyme pad is for.

Microscopic urine sediment counts the cells directly, per high-power field. That count is more specific and is done in the laboratory, not on a home dipstick.

The dipstick result is semi-quantitative and carries no unit:

The scale rises with the amount of enzyme, not with the severity of any condition. A 3+ on a cleanly collected sample deserves a doctor's interpretation, but is not by itself a diagnosis.

Urine Leukocytes: dlaczego ta wartość ma znaczenie

The leukocyte pad is a sensitive but poorly specific test, and those two properties together determine what a result is worth.

Sensitive means it rarely misses genuine inflammation: reported sensitivity sits broadly between 70 and 90 percent. Poorly specific means a positive also arises without infection. Specificity is clearly lower than sensitivity, which is why a positive leukocyte pad on its own is weak evidence. It mainly rules out; it establishes little.

The result gains considerable value alongside nitrite. Nitrite appears when certain gut bacteria convert nitrate in urine, and it behaves in exactly the opposite way: insensitive, but highly specific. Leukocytes positive and nitrite positive in someone with pain on passing urine and urgency makes a bacterial urinary tract infection likely. Leukocytes positive with nitrite negative leaves the question open, partly because not every organism converts nitrate.

Clinically the most interesting pattern is detectable leukocytes with a negative culture: sterile pyuria. That calls for a different line of thought rather than another urine test. Explanations include a recently treated urinary tract infection, a sexually transmitted urethritis such as chlamydia, interstitial cystitis, kidney stones, interstitial nephritis often triggered by medication, and more rarely tuberculosis. In a young adult with pain on passing urine and a negative culture, testing for a sexually transmitted infection is a more logical next step than repeating the urine culture.

The result establishes no diagnosis and is never on its own a reason for antibiotics. That judgement belongs to a doctor, weighing the result together with your symptoms and history.

Urine Leukocytes: kiedy warto wykonać badanie?

The test belongs with symptoms pointing at the urinary tract: pain or burning on passing urine, needing to go more often, urgency without result, cloudy or strong-smelling urine, or lower abdominal pain. It is also requested as part of a broader urine screen, for example when checking kidney function or assessing unexplained fever.

Collection technique determines reliability more than the strip does. Collect a midstream sample: let the first flow pass, collect after that, and stop before the bladder is empty. Clean the area first. A morning sample is the most concentrated and therefore the most informative, though note the limitation under the results below. Deliver the sample to the laboratory within two hours or keep it refrigerated; in urine left standing, cells break down and bacteria continue to grow.

Avoid testing during menstruation. Blood and discharge will almost always enter the sample and make the result unusable.

With fever, chills, flank pain or nausea, contact a doctor the same day. That picture may fit a kidney infection and does not wait for a dipstick.

A negative result is the expected result and causes no symptoms. It means no leukocyte esterase was detectable in this sample, at this moment. A negative does not entirely exclude urinary tract inflammation, and there are circumstances in which it is falsely reassuring. High-dose vitamin C suppresses the reaction on the pad. Highly concentrated urine, a high glucose level and a high protein level do the same, as do some antibiotics including tetracyclines and cefalexin. If symptoms persist while the pad stays negative, contact your doctor again rather than relying on the result.

Wysoki poziom

A positive result causes no symptoms in itself; symptoms come from the underlying cause. With a bladder infection these are usually pain or burning on passing urine, going more often, urgency without result and sometimes cloudy urine. A positive pad without any symptoms does occur, and in older adults it is common: detectable pyuria without symptoms is usual at that age and is not by itself a reason for treatment. A positive can also arise from sample contamination, from vaginal discharge, or from inflammation higher in the urinary tract. Fever, chills, flank pain or nausea alongside a positive result should be discussed with a doctor the same day.

The biggest gain with this test is not lifestyle but collection technique. Contamination is by far the most common reason for a falsely positive result.

Clean the area before collecting and take a midstream sample. Let the first flow pass: it washes out cells and bacteria from the urethra that say nothing about the bladder. Use a clean container and do not touch the inside. Do not collect during menstruation.

Deliver the sample to the laboratory within two hours, or keep it in the fridge. In urine left at room temperature leukocytes break down and bacteria keep growing, and both shift the result.

If you take a high-dose vitamin C supplement, mention it on the request form. It is a well-known cause of a falsely negative pad and the result is hard to interpret without that context. The same applies to antibiotics you are taking or have just finished.

Drink normally in the run-up to the test. Drinking a great deal dilutes the sample, and very concentrated urine after a day of drinking little works the other way. Ordinary intake gives the most usable sample.

What is the difference between leukocytes in urine and leukocytes in blood?
They are two different tests on two different samples. Blood leukocytes give a count per litre and reflect your immune system across the whole body. Leukocytes in urine are a local finding and point to inflammatory activity in the urinary tract. An abnormal urine result does not mean your blood count is abnormal, and the reverse is equally untrue.
Why is my dipstick positive while the sediment shows nothing?
Because the strip does not measure cells but leukocyte esterase, an enzyme the cells release. That enzyme stays detectable after the cells themselves have broken down, for example in dilute urine or a sample left standing too long. The microscope counts only intact cells and finds few. Both results can therefore be correct at once.
What does the nitrite test add to the leukocyte result?
The two complement each other because their properties are opposite. The leukocyte pad is sensitive but poorly specific; nitrite is insensitive but highly specific. Both positive in someone with symptoms makes a bacterial bladder infection likely. Can a poorly collected sample affect the result?
Yes, and it is the most common cause of a falsely positive result. Vaginal discharge, vulvovaginitis, trichomonas and a sample not taken from the midstream all produce a positive pad without a bladder infection. Squamous epithelial cells in the same sample are the clue that contamination is playing a part.
What is sterile pyuria?
Sterile pyuria means detectable leukocytes in urine with a negative culture. It calls for a different line of thought rather than another urine test. Known causes include a recently treated urinary tract infection, a sexually transmitted urethritis such as chlamydia, interstitial cystitis, kidney stones, medication-related interstitial nephritis and, more rarely, tuberculosis. Always discuss this result with a doctor.
When should a positive result be discussed the same day?
With fever, chills, flank pain, nausea or vomiting. That combination may fit a kidney infection and should be assessed the same day. The same applies in pregnancy, with reduced immunity, and when symptoms persist despite treatment. A dipstick is not a reason to wait in those situations.
