Squamous epithelial cells are large, irregularly shaped cells with a small nucleus. In a urine sample they are thought to come from the genital surfaces, according to a 2020 US study, and according to a 2005 US review their presence suggests contamination of the sample.
In that 2020 study, in an emergency department, the presence of squamous epithelial cells went together with less accurate other urine values; few or no cells is the expected result. How many of these cells are in the urine can only be counted with a urine test.
Certe counts squamous epithelial cells with a flow cytometer and gives below 1 per microlitre (µL) as the reference value, for all ages and both sexes.

The squamous epithelial cell test in urine counts how many of these cells are in a urine sample. A flow cytometer counts cells per volume, at Certe per microlitre (µL), and according to the NVKC, the Dutch society for clinical chemistry, it is much more accurate at measuring cell numbers per volume than the microscope, which counts cells per field of view.
For a urine sediment, the undissolved particles in urine, a laboratory looks with a microscope according to the NVKC. A urine strip usually comes first, and only when it is positive is the sediment examined.
Squamous epithelial cells in a urine sample suggest contamination, according to a 2005 US review. In a 2016 US study of adults in an academic hospital, with 19,328 records, the squamous epithelial cell count was however a poor predictor of a contaminated urine culture.
In that study of adults, squamous epithelial cells may however predict poorer performance of the usual urine values in showing bacteria in the urine.
According to the NVKC, the doctor requests a urine test, in which squamous epithelial cells can also be counted, for symptoms that point to kidney disease and urinary tract infections, for example fever, abdominal pain, back pain, passing urine often, pain when passing urine, blood in the urine or cloudy urine.
Few or no squamous epithelial cells in the urine is the expected result: Certe gives below 1 per µL as the reference value.
A urine sample with many squamous epithelial cells is possibly contaminated, according to a 2005 US review. In a 2016 US study of adults, in samples whose culture was not contaminated, a urine test predicted bacteria in the urine less well with more than 8 squamous epithelial cells per low-power field.
For a urine sample for squamous epithelial cells and the other urine values, the NVKC advises washing first, so that no bacteria or skin cells get in. According to the NVKC, the best sample is a portion caught after first passing a little urine into the toilet, preferably the second morning urine, after cleaning the penis or vagina with water, without soap.
According to a 2005 US review, cleansing beforehand, often recommended in women, has no proven benefit; in a study the review cites, 32 percent of samples were contaminated with cleansing and 29 percent without.
