Basophils, in full basophilic granulocytes, are one of the five types of white blood cell of the immune system. They hold substances, such as histamine, that are released in an allergic reaction and can thereby cause the symptoms of such a reaction; according to the NVKC they normally make up only 0 to 2 percent of all white blood cells.

A raised count can occur with allergic conditions, inflammatory conditions and some forms of leukaemia, and the count falls after corticosteroids are given. Only a blood test shows how many basophils are in the blood.

Star-SHL uses 0.0 to 0.15 × 10^9/l for basophils. Unilabs from age 15, Certe for adults and Diagnostiek voor U with no age given use an upper limit of 0.2 × 10^9/l.

The test counts the number of basophils in a litre of blood, expressed in × 10^9/l, billions of cells per litre. According to the NVKC, the count is always done as part of a full blood test. It is part of the white cell differential: the split of the white blood cells over the five types.

UMC Utrecht reports a variation from measurement to measurement of 22 percent at 0.15 × 10^9/l for its own analyser, rising to 31 percent at 0.03 × 10^9/l. According to its own test listing, Star-SHL assesses the blood under a microscope when the differential is abnormal.

Basophils play a role in allergic reactions and, according to the NVKC, occur mainly in tissues where those reactions take place, such as the skin, the nose and the lungs. Because they are mainly active in allergically affected tissues, their number in the bloodstream says little about those allergic reactions, according to the NVKC.

In chronic myeloid leukaemia, a form of blood cancer, a raised basophil count is one of the criteria for the diagnosis according to a review, and according to the NVKC the eosinophil count is often raised too in leukaemia.

For reading the result it helps to know whether someone was given corticosteroids, because those lower the basophil count.

A low basophil count always falls within the adult range: Star-SHL and Certe use a lower limit of zero, and Unilabs and Diagnostiek voor U give no lower limit.

A truly lowered count is rare according to a review. Known situations with a low count are the administration of corticosteroids, active chronic hives and active lupus (SLE), an autoimmune disease.

A normal value for basophils at Star-SHL is 0.0 to 0.15 × 10^9/l. Unilabs uses an upper limit of 0.2 × 10^9/l from age 15, Certe 0 to 0.2 × 10^9/l for adults and Diagnostiek voor U an upper limit of 0.2 × 10^9/l. According to the NVKC, basophils normally make up 0 to 2 percent of all white blood cells.

What does a high basophil count mean?

According to a US review, a high basophil count can occur with allergic and inflammatory conditions, and according to the NVKC it can also result from various types of leukaemia; the count can also be raised after exposure to toxic substances or radiation, according to the NVKC. A rise without other abnormalities is rare according to that US review.

Is a basophil value of 0.0 a problem?

For adults, a basophil value of 0.0 × 10^9/l falls within the range of Star-SHL, Unilabs, Certe and Diagnostiek voor U, because their lower limit is zero or absent. A truly lowered count is seen, among other things, after corticosteroids are given.

How large is the measurement variation of a basophil count?

The measurement variation of a basophil count on UMC Utrecht's own analyser is 22 percent at 0.15 × 10^9/l and 31 percent at 0.03 × 10^9/l.
